Order Form

• Submit orders to the Animal Care Manager (or, if unavailable, to the Animal Care Assistant Manager).

Transfer Request Form

• Please review the Policy for Transfer of Animals prior to form submission.
• Email transfer requests to Vivarium@charlotte.edu. Alternatively, you may print your form and place it in the Vivarium’s physical inbox, located beside the Assistant Manager’s office.
• If the transfer is approved, the PI will be notified. Vivarium staff will update animal numbers in the database. The PI (or research staff) is responsible for updating the protocol number on cage cards.
